Ellsworth Corporation's employee benefit program provides employees with several benefit options and the flexibility to choose the benefits that fit their needs, finances and lifestyles.
Each employee will receive a 60 day orientation period. During orientation period, the new employee is not eligible for any company benefits, including holiday, sick time or insurance benefits.

Physician's office visit: $25.00 co-payment.
In PPO Network: Deductible: $1000 (Base Plan)

2. Prescription Drugs

Prescription drug co-payment: $15.00 generic drug co-pay; $25.00 select brand name co-pay; $50.00 additional brand name co-pay; 25% for high technology drugs.
3. Life Insurance

Ellsworth employee Basic Life & AD&D Insurance is $25,000.00.
Cost is $.39 per paycheck.
4. Dental Insurance
Mutual of Omaha (administered by United Concordia)
Employee cost: 20% of premium.
Employee contribution/per paycheck: $2.60 Single and $7.12 Family.
5. Disability Insurance
Mutual of Omaha
Participation for all Ellsworth employees is highly recommended. When disabled, short-term disability plan pays 60% regular weekly earnings (up to a maximum of $500 per week). Short-term cost: $.27 per $10.00 of weekly covered payroll. (Paid 20% by employee, 80% by company). When disabled, long term disability plan pays 60% of monthly earnings (up to a maximum of $6000 per month). Long-term cost: $.36 per $100.00 of monthly earnings. (Paid 20% by employee, 80% by the company).
6. Optional Life Insurance
MetLife
Optional life insurance is available for you and your dependents through a term policy. Costs vary depending on age. Evidence of Insurability will be required for all enrollees.
7. Profit Sharing Retirement 401(k)
401(K) Savings Plan – In order to be eligible, an employee must be 21 years of age and complete two months of service.
Dates of Enrollment: First of each month after 60 days of service.
Employee Contributions: Contributions are voluntary; processed through payroll deduction. You may contribute from 1% to 25% of total pay. Contributions can be changed on a monthly basis>
Company Contributions: Matching: The company will match 50 cents to your dollar, on the first 6% of your contributions.
8. Employee Assistance Program (EAP)
Provided by Crites and Associates
The Employee Assistance Program (EAP) is sponsored by Ellsworth Corporation and provides free, confidential problem assessment and referral services for employees and family members with a broad range of personal problems including emotional/behavioral, family and marital, alcohol and/or drug, financial, legal and other issues. This is strictly a voluntary program.
9. Wellness
Ellsworth Corporation sponsors a wellness program in an effort to create an environment supportive of positive wellness and healthy habits. The objectives of the program are to introduce, educate and provide support with nutrition and weight management, physical activity, stress, self care, and safety issues. Wellness Program activities include posters/fliers/pamphlets/newsletters, general health screenings, health risk appraisals, speakers, videos, medic first aid training, wellness walks, etc.
10. Social Functions
Ellsworth Corporation entertains employees during the year at a variety of social events, including picnics and holiday parties. Although these events are paid for by the company, attendance is completely voluntary.
11. Service Awards
Ellsworth Corporation acknowledges and rewards employees for their length of service. Service Awards are presented to employees who have achieved five, ten, fifteen, etc., years of service with Ellsworth Corporation. The service award consists of a monetary award and a personalized gift.
12. Tuition Reimbursement
Full-time employees with at least one year of service may request to take job-related, undergraduate courses at accredited institutions (outside of normal working hours). Each request for tuition assistance will be considered on a case-by-case basis.
13. Adoption Assistance
All full-time employees with more than one year of continuous service are eligible for financial assistance (up to $5,000) and paid time off when adopting a child. The child must be under 18 years of age and not already related to the employee or employee's spouse.
14. Flexible Spending Accounts
Process Works
Allows employees to deduct specific amounts from their paycheck on a pretax basis for reimbursement for Health Care-related expenses (up to $4,000 per calendar year) and Dependent Care Expenses (up to $5,000 per calendar year). Eligible first of the month following the 60 day orientation period.
15. Vacations
The vacation policy is designed to reward employees for continued employment. Vacation eligibility is calculated on a calendar year basis as follows:

16. Sick Time
5 Days (or 40 hours) of paid sick time is available to each employee per year. The employee must have completed the 60-day orientation period to be eligible. Perfect Attendance awards are given to non-exempt employees who work a full calendar year.
17. Holidays
Ellsworth Corporation observes the following ten (10) paid holidays per year. Typically, the Holidays follow the schedule below. When necessary, management may change this schedule to accommodate business needs.

18. Part-Time Employee Benefits
Employees who work more than 30 hours per week but less than 36 hours per week will be eligible for medical/dental/life/disability insurance, profit sharing, holiday pay (6 hours per holiday), vacation pay at a rate of 75% of the vacation allocated for a regular full-time employee with equivalent service time, sick pay of 30 hours per calendar year, EAP, adoption assistance, Savings Bonds, and the Flexible Spending Accounts.

Please Note: All insurance (medical, dental, disability and life) becomes effective the first of the month following the 60 day orientation period. For Example, if you begin employment on January 10th, your insurance becomes effective April 1st.
